PARAFRASEAR EN TODA LA HISTORIA, lo que quiere decir mientras tomamos nota debemos REPETIR lo que nos dice el paciente, el propósito es doble: Asegurarnos de tener los datos bien, para el ICE y los puntos de CIS por parafrasear.

TRANSITION: “Now I am going to ask you some questions about your health in the past” / “past medical history”/ “is that ok with you?”
Para el Past Medical History (PMH): “PAM HUGS FOSS” esto cubre casi todo
P >rior episodes of the CC, past medical history, si el paciente tiene Diabetes o HTA y si toma medicacion, Preguntar por Hipercolesterolemia tambien.
A > llergy hx ( a medicaciones, comida, ambiente, etc.)
M> edications Px is taking, hay que incluir los suplementos, y la pregunta mágica “ Do you take anything over the counter?” (OTC). A los pacientes en EEUU se si se les pregunta si toman alguna medicacion siempre van a decir que no, pero si se les pregunta OTC ahi si van a responder por cosas que se compran en los super sin receta, tipo ibuprofeno, paracetamol, antihistaminicos, etc
H> ITS : Hospitalizaciones, Internaciones, Trauma, Surgery.
U> rinary problems
G> I problems (constipación, diarrea, “pencil thin stools”, weight changes, eating habits, diet)
S> leep problems (sleep pattern and changes)
F> amily history
TRANSITION: “Now I am going to ask some questions about your female health is that OK with you?”

TRANSITION: “Now I am going to ask some questions about your sexual health. Please let me assure you, this will be kept absolutely confidential”
S> exual hx (activity, condom OCP use, AIDS test, prior STD’s)
TRANSITION: “Now I am going to ask you some questions about your social life”
S> ODA. Smoking/Occupation/ Drugs/ Alcohol + CAGE. Con respect al CAGE (Cut drink, Annoyed when asked about drinking, Guilt when thinking about it, Eye opener drink early in the morning), no conviene preguntarlo de una, primero sobre los habitos, hay que quitar NUMEROS, cuantos vasos, cuantas botellas, y la frecuencia, despues de eso si el numero es o exagerado o no convincente podemos ir al CAGE.
El occupation es una buena pregunta para dejar para hablar mientras se lavan las manos
SUMMARIZE: “Let me summarize Mr Kent, so far you told me that you had this and this and this. Did I get all right? OK, to narrow down my diagnosis I have to run a very brief physical exam, is that OK with you?, Now I am going to wash my hands”
Mientras nos lavamos las manos es importante que nos mantengamos hablando, de la ocupacion, o de la vida social, o de alguna otra cosa que nos hayamos olvidado de preguntar.
