Aquí hay que hacer por lo menos 5 cosas:
SUMMARIZE: “OK Mr Kent, because THIS and THIS and THIS that you told me and THIS and THIS I found on physical examination I believe…”
DD, Differential Diagnosis “ .. you might be having THIS condition, or THIS or THIS” dos por lo menos hay que dar.
WORK UP: “… to make sure this is what is troubling you we will need a picture of your chest/bloodwork/etc.” IMPORTANTE: no usar terminologia medica, si se nos sale algo muy esotérica sin que nos demos cuenta tenemos que explicarlo de inmediato, antes de que el paciente pregunte que es
REASSURANCE: “I assure you this is a perfectly treatable condition/ you will be able to carry on with your life normally again/etc, etc / We have an outstanding team working on this condition all day/etc. etc.” cualquier cosa que podamos decir para tranquilizar al paciente cuando le decimos su diagnostico, y de acuerdo a cada condicion.
COUNSELING: “.. and regarding your smoking habit/ drinking habit/ IV drug use , is my duty as your physician to warn you about the risk you are taking and your current behavior could lead you to this and this worse/deadly conditions if you don’t change this. We have an excellent support group available at the clinic I will leave the contact with the nurse.” Algo por el estilo, hacerle claro que lo que hace es malo para su salud, y brindarle opciones para salir adelante.
Y por ultimo, Do you have any question?
El cierre o closing bien hecho toca por lo menos 5 a 6 puntos de CIS, por ende ante sacrificar parte del examen físico y parte del closing, se sacrifica parte del examen físico, no hacer closing en dos o más encuentros casi asegura un FAIL en CIS y por ende en todo el examen.
ME QUEDE SIN TIEMPO, QUE HAGO?? La clásica respuesta de tocarse el bolsillo y decir “I have been paged, its an emergency I will be right back” no queda mal, por más que oficialmente todo lo que hagamos después de que llegue el tiempo es irrelevante para nuestra nota, por lo menos deja buena impresión en el SP ;).
